Support Designed Around the Whole Person
Serenity Changes Foundation is developing compassionate, person-centered programs for seniors, veterans, people with disabilities, and low-income individuals and families throughout Chattanooga and Hamilton County. Our programs are designed to promote housing stability, independence, dignity, safety, and meaningful connections to essential community resources.
Our Core Programs
Services are based on individual needs, available resources, program eligibility, and organizational capacity. Some programs remain in development as Serenity Changes Foundation secures property, funding, partnerships, and additional community support.
Supportive Housing and Housing Stability
Our supportive-housing program is being developed to help eligible seniors, veterans, people with disabilities, and low-income individuals obtain safe, stable, and affordable housing. Participants will receive individualized support intended to strengthen housing stability, reduce barriers, and promote long-term independence.
Independent Living Assistance
Independent-living support will help participants strengthen the skills needed to live safely and confidently. Depending on individual needs and program availability, assistance may include daily-living skills, household organization, meal planning, appointment preparation, community-resource connections, and coordination of nonmedical personal support.
Case Management and Benefits Navigation
Participants may receive person-centered case management to identify needs, establish goals, develop an individualized service plan, and coordinate appropriate resources. Benefits navigation may include assistance understanding and accessing eligible healthcare, housing, food, transportation, veterans, disability, and income-support programs.
Transportation and Appointment Coordination
Transportation assistance will be designed to help eligible participants attend medical appointments, benefit-related meetings, essential errands, and approved community activities. Services may include transportation scheduling, appointment reminders, trip coordination, and connections to available transportation providers.
Meals and Nutritional Support
Our planned nutritional-support program will help eligible seniors, veterans, people with disabilities, and low-income individuals and families access healthy meals and essential food resources. Support may include meal delivery, grocery assistance, nutrition education, and connections to community food programs, based on available funding, partnerships, and individual needs.
Low-Income Family Stabilization
Our planned family-stabilization services will support eligible low-income individuals and families experiencing financial hardship, housing instability, or difficulty accessing essential resources. Assistance may include individualized referrals, benefits navigation, connections to food and housing resources, transportation coordination, and practical support designed to help families achieve greater stability and independence.
Senior, Veteran, and Disability Support
Serenity Changes Foundation is developing coordinated support for seniors, veterans, and people with disabilities who may face barriers to housing, transportation, benefits, daily living, and community participation. Services may include individualized resource coordination, benefits navigation, independent-living assistance, transportation support, and referrals to trusted community partners based on eligibility, availability, and individual needs.
Planned Eligibility Guidelines
Eligibility will be determined according to each program’s requirements, available funding, and organizational capacity. Serenity Changes Foundation plans to serve eligible residents of Chattanooga and Hamilton County whose household income is at or below 80% of the Area Median Income, with priority consideration for households at or below 50%. Additional requirements may apply depending on the specific service or funding source.
Referral and Intake Process
Individuals, family members, caregivers, healthcare providers, veterans’ organizations, social-service agencies, and community partners may contact Serenity Changes Foundation regarding future services or referrals. Our team will review each inquiry, discuss the individual’s needs, and provide appropriate information or community-resource connections. Submitting an inquiry does not guarantee eligibility, enrollment, housing placement, or immediate assistance.
